- INFORMATION AND INSTRUCTIONS INCLUDED ON FORMS. BOTH FORMS ARE REQUIRED. PRINTED COPIES AVAILABLE AT THE ADMINSTRATIVE OFFICE.

Are you 18 years or older? Do you have a High School Diploma or GED? Are you planning on attending an accredited university or college and take a minimum of 6 credit hours of study?
If yes, you may be eligible for financial aid!
Please submit the following completed applications:
- HIGHER EDUCATION SCHOLARSHIP APPLICATION (Completed by applicant)
- NEEDS ANALYSIS SUMMARY (Completed by applicant AND Financial Aid Office)
- EDUCATION REPAYMENT AGREEMENT (Completed by applicant)
Each applicant must also provide:
- Copy of High School Diploma or GED Certificate
- Personal letter outlining Educational Goals
- Letter of acceptance from the College/University
applications will be reviewed for completeness and then presented to the Summit Lake Tribal Council for approval
